There’s no reason for a form to be ugly, boring, or hard to fill out. These forms for pet owners were created so that clinics could post them to their Vetstreet-produced websites or e-mail them directly to their clients. To make them easy for the clinics to sort and file, I designed the forms with simple graphics to differentiate them. And to make it easier on the clinics’ clients, I took the time to make sure there was more than enough room to fill in the longest names or complicated addresses, and then set up each as an interactive PDF.
